it could be you injured the nail at one point and the new nail is growing in under the old nail.
as it grows it pushes the dead nail up and this can take a while to complete.
soak you foot in listerine mouthwash this is an antiseptic and it might help

well, im sure they will numb it for whatever they have to do, it sounds like an ingrown nail, it can happen if you cut them too short, on accident or so, or from a fungus or something... either way, not fun :( sorry you gotta go through that!
